Not trying to be factitious, but it's always a case of pulling the trigger. Here, I'll bet this guy forgot or didn't know enough to take his finger out of the trigger guard after the BG fled. Adrenalin rush causes shaky fingers. The result, one shot foot.

That's one possibility, and probably a good guess. Another might be an attempt at "quick draw" maybe from IWB or pocket carry, and if so, he's lucky all he hit was his foot. God forbid he was carrying in a Thunderwear.
